Remote Patient Monitoring (RPM) is a service used to monitor patients’ chronic and acute conditions using connected medical devices, such as glucose meters, pulse oximeters, and blood pressure machines. These devices remotely collect and transmit patient readings to their clinicians and healthcare professionals. Private insurances and Medicare provide reimbursements for Remote Patient Monitoring.
A Remote Patient Monitoring program involves the following stages
Patient Onboarding
Identify Eligible Patients. Remote Patient Monitoring (RPM) serves patients with chronic and acute conditions, such as hypertension, diabetes, COPD, and COVID-19.
Distribute Devices. Relevant medical devices (e.g., glucose meter, blood pressure machine, pulse oximeter, etc.) are delivered to enlisted patients.
Patient Training & Setup. Patients are provided a demonstration on how to utilize the connected medical devices to transmit health data.
Monitoring Patient Data
Collect Medical Data. As patients utilize their connected medical devices, their blood pressure, blood glucose, oxygen saturation, and/or supplemental physiological data is remotely transmitted to their healthcare provider for review.
Trend Analysis. As daily physiological readings are consistently procured, trends within the patients’ readings emerge. This enables health providers to recognize early signs of potential problems before they become more severe.
Alert Management. Attend to patients transmitting physiological data which exceed pre-established thresholds.
Patient Compliance
Maintain Interest. Patients may need additional encouragement and incentive for taking physiological readings on a consistent basis.
Data Collection. 16 reading days need to be accrued per patient every 30 days in order to successfully receive reimbursement for CPT code 99454.

Collecting Reimbursement. Remote Patient Monitoring (RPM) offers 5 CPT codes which cover patient setup, collecting physiological data from connected medical devices, dedicating time towards reviewing patient data, and interactively communicating with the patient.
Billing Frequencies. CMS Remote Patient Monitoring CPT codes have a disparate set of requirements. CPT Code 99453 is billed once, 99454 is reimbursable on a 30-day cycle once the patient reaches 16 reading days, 99457 is billed once per month, and 99458 can be billed twice per month.
